Evaluation Includes

​

  • Evaluation & Physical Examination with the Doctor

  • Medical Record Review & Assessment

  • Registration in the Office of Medical Marijuana Use Registry

  • Certification for Medical Qualification 

  • Medical Marijuana Treatment Plan 

  • Three 70 - day Medical Marijuana Orders - total of 210 days

  • Ongoing office visits as needed by the patient during the treatment period

Your certification will be good for 210 days

Recertification is required by law every 210 days.
